Abstract


The most common fractures of the spine are associated with the thoracolumbar junction. The goals of treatment of thoracolumbar fracture are leading to early mobilization and rehabilitation by restoring mechanical stability of fracture and inducing neurologic recovery, thereby enabling patients to return to the workplace. However, it is still debatable about the treatment methods. Neurologic injury should be identified by thorough physical examination for motor and sensory nerve system in order to determine the appropriate treatment. The mechanical stability of fracture also should be evaluated by plain radiographs and computed tomography. In some cases, magnetic resonance imaging is required to evaluate soft tissue injury involving neurologic structure or posterior ligament complex. Based on these physical examinations and imaging studies, fracture stability is evaluated and it is determined whether to use the conservative or operative treatment. The development of instruments have led to more interests on the operative treatment which saves mobile segments without fusion and on instrumentation through minimal invasive approach in recent years. It is still controversial for the use of these treatments because there have not been verified evidences yet. However, the morbidity of patients can be decreased and good clinical and radiologic outcomes can be achieved if the recent operative treatments are used carefully considering the fracture pattern and the injury severity.

Abstracto
Las fracturas más comunes de la columna vertebral están asociadas con la unión toracolumbar. Los objetivos del tratamiento de la fractura toracolumbar están dando lugar a la movilización precoz y la rehabilitación mediante la restauración de la estabilidad mecánica de la fractura y la inducción de la recuperación neurológica, lo que permite a los pacientes a volver al lugar de trabajo. Sin embargo, todavía es discutible acerca de los métodos de tratamiento. Las lesiones neurológicas debe identificarse mediante un examen físico completo para el motor y el sistema de los nervios sensoriales con el fin de determinar el tratamiento adecuado. La estabilidad mecánica de fractura también debe ser evaluada por radiografías simples y la tomografía computarizada. En algunos casos, se requiere formación de imágenes de resonancia magnética para evaluar la lesión del tejido blando que implica la estructura neurológica o complejo del ligamento posterior.Basándose en estos exámenes físicos y los estudios de imagen, se evalúa la estabilidad de la fractura y se determina si se debe utilizar el tratamiento conservador o quirúrgico. El desarrollo de instrumentos han llevado a más intereses sobre el tratamiento quirúrgico que salva segmentos móviles sin fusión y en la instrumentación a través enfoque mínimamente invasiva en los últimos años. Todavía es controversial para el uso de estos tratamientos porque no se han verificado evidencie todavía. Sin embargo, la morbilidad de los pacientes se puede disminuir y buenos resultados clínicos y radiológicos se puede lograr si los tratamientos quirúrgicos recientes se utilizan cuidadosamente teniendo en cuenta el patrón de fractura y la gravedad de la lesión.
