Resumen

IntroducciónEl abordaje anterior vertebral conocido como mini-open fue introducido hace unos años para el tratamiento quirúrgico de la patología del raquis. Dicho abordaje permite la exposición directa de las estructuras anteriores, cuerpo y disco intervertebral, la descompresión anterior del saco dural y la reconstrucción y/o estabilización de los niveles de interés con un sistema adecuado. En el presente trabajo presentamos nuestra experiencia en mini–open anterior spine surgery (MOASS) en el abordaje anterior del raquis para tratamiento de patología del raquis toracolumbar.
Material y métodosEn el periodo entre enero de 2004 y julio de 2011 hemos realizado 74 abordajes anteriores de columna mediante cirugía abierta. En 38 casos utilizamos la técnica MOASS a distintos niveles: torácico, lumbar e infraumbilical extraperitoneal.
ResultadosNo tuvimos mortalidad quirúrgica ni postoperatoria, ni déficitsneurológicos añadidos derivados del acto quirúrgico, en ninguna de las técnicas (clásica versus MOASS). Las complicaciones fueron escasas y corregidas con el adecuado tratamiento.
ConclusionesMediante la técnica MOASS hemos realizado la corrección quirúrgica de lesiones vertebrales que pueden afectar al raquis toracolumbar, con resultados similares a los obtenidos mediante el abordaje clásico y ventajas reseñables.

Abstract

IntroductionThe anterior spine approach known as «mini-open» was introduced a few years ago for the surgical treatment of spine diseases. This approach allows the anterior, body and intervertebral disc structures to be exposed, as well as the anterior compression of the dural sac and the reconstruction and/or stabilisation of the levels of interest with an appropriate system. In the present article we present our experience in mini–open anterior spine surgery (MOASS) approach in the treatment of lumbar spine diseases.
Material and methodsWe performed 74 anterior spine approaches using open surgery between the period January 2004 and July 2011. In 38 cases we used the MOASS technique at different levels: thoracic, lumbar, and infraumbilical extraperitoneal.
ResultsThere were no surgical or post-operative deaths, or further neurological deficits arising from the surgical procedure in any of the techniques (classic versus MOASS). The few complications were corrected with the appropriate treatment.
ConclusionsUsing the MOASS technique we have performed corrective surgery on spine injuries that could affect the thoracic spinal column, with similar results to those obtained using the classic approach and with obvious advantages.

 Spinal lumbar stenosis is a disease that occurs mainly between the 5th and 7th decade of life and can be congenital or acquired. The latter has many etiologies, but a degenerative cause is the most common. Stenosis is a narrowing of the spinal canal diameter caused by many factors such as bulging discs, hypertrophy of flavum ligament, facet capsule thickening and osteophyte formation. The classical symptom of the disease is sciatic pain, that improves with lumbar flexion and worsens with ambulation. Neurological examination is often normal and the most useful imaging test is magnetic resonance imaging (MRI). Conservative management consists in non-steroidal anti-inflammatory drugs, muscle relaxants, physical therapy and epidural - radicular infiltrations. Spinal infiltrations have a proven efficacy for pain management. A good result of this therapy predicts a favorable outcome after surgery. Surgical treatment consists in decompression with or without lumbar fusion. The addition of an arthrodesis is recommended for degenerative spondylolisthesis, correction of deformities, recurrent spinal stenosis with instability, sagittal or coronal imbalance and adjacent segment disease.

La raquiestenosis lumbar se define como una disminución del área del canal raquídeo que puede comprimir las raíces nerviosas. La incidencia en las imágenes de este cuadro va de 1,7% a 13,1%1, sin embargo, la incidencia clínica es menor y genera sus síntomas con mayor frecuencia entre la 5° y la 7° década de la vida. Su causa principal son los cambios degenerativos de la columna vertebral. Se produce con igual frecuencia entre hombres y mujeres, excepto cuando esta patología se asocia a espondilolistesis, la cual es de mayor incidencia en mujeres. No hay asociación con actividad laboral o física.

Resumen

Dada la gran prevalencia del dolor lumbar, la cirugía de columna es una intervención cada vez más frecuente. Existen múltiples procedimientos quirúrgicos disponibles, de los que la laminectomía y discectomía son las intervenciones más frecuentes. En un 10-40% de los pacientes intervenidos, el dolor lumbar que ocasionó la intervención puede recurrir o no solucionarse completamente, lo que se incluye dentro del síndrome de cirugía fallida de columna. Hay múltiples causas que pueden ocasionar este síndrome, aunque frecuentemente es de etiología multifactorial y no deben confundirse con los hallazgos normales en columnas postoperadas. La decisión de la técnica de imagen a realizar dependerá del tipo de cirugía, la utilización de material ortopédico metálico y la sospecha clínica. El conocimiento de las ventajas y limitaciones de las distintas técnicas de imagen disponibles es esencial para la óptima valoración de estos pacientes, especialmente tras cirugía con instrumentación donde serán necesarios ajustes técnicos para minimizar el artefacto producido por estos materiales.

Abstract

Given the prevalence of low back pain, surgical interventions on the lumbar spine are becoming more common. Among the many surgical procedures available for these interventions, the most common are laminectomy and discectomy. In 10 to 40% of patients who undergo surgical interventions on the lumbar spine, low back pain is not completely alleviated or it recurs, and these cases fall into the category of «failed back surgery syndrome». This syndrome can have many different causes and multiple factors are often involved. It is important not to confuse the normal postoperative findings with those specific to failed back surgery syndrome. Deciding which imaging technique to use will depend on the type of surgical intervention, whether metallic orthopedic material was used, and the clinical suspicion. It is essential to know the advantages and limitations of the available imaging techniques to ensure the optimal evaluation of these patients, especially after interventions carried out with instrumentation to minimize the artifacts due to these materials.

Abstract
Introduction: Fracture-dislocation of the thoracic or lumbar spine often results in severe neurologic deficits if dislocation is significant. However, cases of fracture-dislocation of the thoracic or lumbar spine without neurologic deficits are rarely reported in the literature, and the choice of the treatment has been controversial.
Case Report: Two female patients, aged 27 and 35 years, were injured in motor vehicle accidents and did not have neurological deficits except for slight numbness in the thighs in one case. Radiologic examinations showed nearly complete fracture-dislocations at T7-8 and L1-2, respectively. In both cases, subtotal corpectomies and anterior reconstructions using Kaneda devices were performed after laminectomy in the lateral decubitus position. No neurological deterioration was observed after surgery.
Conclusion: Anterior subtotal corpectomy and reconstruction combined with posterior decompression is a good option for these cases to restore the alignment and the stability of the spine.

Resumen:
Introducción: La fractura-luxación de la columna torácica o lumbar a menudo resulta en déficits neurológicos graves si dislocación es significativo. Sin embargo, los casos de fractura-luxación de la columna torácica o lumbar sin déficits neurológicos son raramente reportados en la literatura, y la elección del tratamiento ha sido motivo de controversia.
Caso Clínico: Dos pacientes de sexo femenino, de entre 27 y 35 años, resultaron heridos en accidentes de vehículos de motor y no tenía déficits neurológicos excepto ligero entumecimiento en los muslos en un caso. Exámenes radiológicos mostraron fracturas-luxaciones casi completos en T7-8 y L1-2, respectivamente. En ambos casos, corpectomía y anterior reconstrucciones subtotal utilizando dispositivos Kaneda se realizaron después de laminectomía en la posición de decúbito lateral. Sin deterioro neurológico se observó después de la cirugía.
Conclusión: Anterior corpectomía subtotal y reconstrucción combinada con descompresión posterior es una buena opción para estos casos para restaurar la alineación y la estabilidad de la columna vertebral.
